  2. Your full name as it appears on University records. Do not use initials. Indicate your previous degrees conferred.
  3. Dissertation is used by doctoral students; thesis is used by master's students.
  4. Indicate the degree objective for which you are submitting this manuscript.
  5. This is your official major as it appears on your University records.
  6. Type in the name of chair under the signature line. Type in the names of the two readers under the second and third line. Add additional signature lines if necessary.
  7. Indicate the year in which your degree will be officially conferred.
  8. The title page is always numbered as small Roman numeral -i-.
